It was VERY most likely from Trump himself. It's from 2003 and the documentation was being held by the estate attorney(s). 2003 was before Trump was ever involved in politics whatsoever, so unlikely somebody signed his birthday book in 2003 with a fake Trump signature. 

Evidence overwhelmingly points to Trump here, regardless of what some rigged/biased analysis indicates.
