Any time you have someone this old with several comorbidities demonstrating symptoms of advanced aging and decay both physically and mentally you have to be ready for them to go at any second. Especiallllly with congenital heart failure.

Is it mental decline, or just more of the standard BS, that led him to recently claim “they call me the President of Europe, it’s an honor”.

It's likely a combination of things, none of which are uncommon for a 79 year old man. Likely on blood thinners for heart issues (see his swollen lower legs), which could be anything from your typical high blood pressure to congestive heart failure. Probably getting blood drawn or some sort of injection in his hand because they have trouble with the vein in his elbow (also pretty common). He could die tomorrow or in 15 years. In other words, he's an average 80-year old American man.

Five years ago he had several comorbidities. According to science if you were > 65 with one or more you were doomed.
